To help make your first visit to our practice as smooth as possible, please review the information below. You'll find the practical information you need such as a map and directions to our office hours payment policies and more.   You can also save time by printing out and completing the new patient forms in advance.

Office Philosophy

We strive to treat others as we ourselves would like to be treated.

What to wear

We suggest you where a short sleeve shirt with a relaxed fit and an open collar to your visit. This makes your head and neck exam much more comfortable and complete.

What to bring to your visit

Insurance information: Please bring your insurance card(s) and drivers license or other form of identification.

Copayment: Our contractual obligations with many insurance companies require we collect copayment at the time of service. For your convience, we accept cash, personal checks, Mastercard, and Visa.  Failure to bring your copayment may force us to reschedule your appointment for a later date. Please review your insurance policy or contact your insurance company for details on what your particular plan requires.

Medications: Please bring a list of all your medications and their dosages with you.

CT and MRI scans: If you have had a recent CT or MRI scan, please contact the facility where the study was performed and ask for a copy of the scan(s) to be given to you to bring to your appointment.  Be sure to ask for the complete study and not just the typed report.  Dr. Benson may need to personally review the images to help better treat you.
